Preliminary Review Form

Instructions and Important Information

How to process the online Preliminary Review Form below:

1. Complete your contact information.

2. Select your ancestor from John Dunham's children at the dropdown list.

3. Fill out the remainder of the form beginning at the 3rd Generation with the name of the son or daughter of the individual you chose, and then fill in who they married. Then list the 4th Generation son or daughter and who they married. Continue this process through the generations down to yourself AND your spouse (if applicable). We will compare your proposed lineage with the lineage records in our database records to determine what portion of your line is already documented. You will receive a report on the findings with instructions on how to proceed.

Please expect to wait three to six weeks for your response. Our report will be sent to you in an email from historian@johndunhamsociety.com

Please add historian@johndunhamsociety.com to your email contacts or safe senders list to prevent your preliminary review results from being marked as spam. If you do not receive your results in six weeks, be sure to check your spam/junk-mail folder as some email systems may route our email to that folder.

There is a non-refundable $35.00 fee per inquiry for this service. A credit card or a PayPal account is required for instant processing.

This Preliminary Review Form is for those individuals who want a review conducted on their proposed lineage to Deacon John Dunham PRIOR to continuing their lineage research. If you believe you already have documentation that satisfies our requirements for membership, then follow the membership application process listed on our Membership page.
